BSA Health and Medical Records
- Parts A & B
- Required for all Scouting activities less than 72 hours long
- Required for all youth and adults participating in activities
- Colorado Addendum
- Required for youth at all Scouting activities in Colorado
- This is your son's immunization record, which you can obtain from his pediatrician
- Parts A, B & C
- Required for all Scouting activities longer than 72 hours
- Not usually required for Cub Scouts
Other Forms
- BSA Activity Consent Form
- Required for all Scouts attending activities
- Also required for other youth, such as siblings, but not adults
- Will be passed out for all events requiring them
Fishing License: Colorado
If your youth (under 16) is fishing and you are touching any of the gear you need to have one. You can buy one online, print out the receipt and have it mailed to you. You can fish with just the receipt for 14 days.
